Worry Free Home Care

Special skill is required to provide psychiatric nursing services in a patient’s home. VNA Community Healthcare’s psychiatric nurses have it and offer it every day. Most of our nurses are certified psychiatric nurses, with one or more years of psychiatric training. We provide patients with support and structure, ensuring medication compliance and promoting stability necessary for independent living.

It begins with a doctor’s order requesting psychiatric services. After we receive a doctor’s orders requesting treatment in the community, we conduct an initial assessment in the home. We create a customized plan of care for each patient. Our extensive background in working with this population allows us to overcome resistance and build rapport. Through ongoing relationships we teach coping and adaptive skills.

A regular medication schedule helps patient independence. By administering medication as often as twice daily, our nurses help patients with chronic mental illnesses live independently. Our nurses also help family members with strategies for caring for a mentally ill family member. We also assist in the management of co-morbidities, including diabetes and obesity.

Our experienced nurses are a critical link with our patients’ doctors. When we detect that medications have become less effective, we work with doctors to develop new treatment plans. We also recommend therapy programs to overcome addictions, including gambling and alcoholism. Our goal is to keep our patients healthy and stable so they can stay out of the hospital and live longer and healthier lives.
