'Hospitals Face Pressure to Avert Readmissions'
Medicare has begun levying penalties on hospitals it says have too many readmissions. This pressure on hospitals is having a ripple affect on physicians in the community and home health care agencies such as VNA Community Health. Our 'Well Right Now" program is in place to address the needs of individuals with chronic illnesses.
Spotlight on New Resources
VNACHC has opened a wellness/resource center for residents of Bella Vista Senior Living Community in New Haven. The site offers on-site wellness programs, fall risk screenings, health coaching, blood pressure screening and caregiver support for residents and their families.
