Did you know there is a new way to monitor your cardiac patients on a daily basis?  Telehealth may be an important part of your patient's health stability.

The cardiac telemonitoring device used by VNA Community Healthcare allows our home health care nurses to “keep in touch” with cardiac patients’ vital signs and symptoms through a home phone line. This daily cardiac telemonitoring gives you and our nurses insight into the health changes of your cardiac patients.

With in-home telemonitoring, patients receive a three-minute checkup every day in the comfort of their own homes through a device that records and reports vital signs. This information is transmitted to specially trained cardiac nurses for review. We are able to catch any irregularity that may pop up between scheduled doctor and home care visits before it escalates into a major medical emergency.

Telemonitoring is one component of complete home health care for cardiac patients that can include physical therapy for rehabilitation and reconditioning, occupational therapy for energy conservation, and access to a dietician.

Consider referring your patients for cardiac monitoring following:

  • A new cardiac diagnosis
  • Post-MI rehabilitation at home
  • Cardiac surgery
  • Congested Heart Failure
  • Coronary Artery Disease N
  • New cardiac medications
